Admission Documents Required

For admission to Trinity School, the following documents must be completed and provided:

  1. Admission Application accompanied with enrollment fee payment
  2. Medical Release Form
  3. Emergency Contact Form
  4. Student Birth Certificate (copy) or other legal verification of the child’s birth date
  5. Immunization Records that are current and up-to-date.  The State of Oklahoma requires evidence of adequate immunization records before any child may be enrolled in any school in Oklahoma.  State requirements follow:
    • 5 doses – DPT  K–8
    • 4 doses – Oral Polio K–8
    • 2 MMR – K–8
    • 3 doses – Hepatitis B PK–5, 7, and 8
    • Chicken Pox Vaccine PK–5 (or written notification that child has had Chicken Pox)
  6. Transfer Records from the previous school attended
  7. Records and Results from any educational testing and/or psychological evaluations
  8. Student Information Form completed by the student’s current teacher(s)
